What this website collects.
This page covers the appointment request form on this site. It is not the practice's HIPAA Notice of Privacy Practices — that is a separate document about your medical records.
What the form collects
When you request an appointment we collect: the location you chose, your preferred date and time, your name, your phone number, your email address, the service you said you were interested in (optional), a short scheduling note (optional, 300 characters), the language you were reading the site in, and the time you agreed to be contacted.
We use it for one thing: to call or email you back about that request.
What it never collects
Date of birth. Insurance member ID. Medications. Lab or test results. File uploads. There is no field anywhere on this site that asks about symptoms, a condition, or a reason for your visit — the service field asks what you are interested in, and the note field asks about scheduling and asks you not to include medical information.
Please do not send medical details through this form. Bring them to your visit, or call the clinic.
Who handles it on the way
The site is hosted by Cloudflare, and the request is stored in a Cloudflare database. Cloudflare Turnstile checks that the form was submitted by a person rather than a bot. The two confirmation emails — one to you, one to the clinic — are delivered by Resend.
We do not sell your details, and we do not share them with anyone beyond the providers above and the practice itself.
How long it is kept
Appointment requests are deleted automatically 180 days after they arrive. That is enforced by a scheduled job, not by someone remembering.
Cookies
This site sets no advertising or analytics cookies. Cloudflare Turnstile may set a short-lived cookie as part of checking the form submission, and the staff area sets a session cookie for signed-in clinic staff only.
Your choices
You can ask us what we hold about your request, ask us to correct it, or ask us to delete it before the 180 days are up. Contact the practice using the details below.
